Intake Coordinator on
Commercial Insured Payors Position Description :
Under the supervision of the Clinical Director, the fully licensed Assessor (LCSW or LPC) performs a variety of clinical and administrative work when it has been determined through an initial screening that the individual has mental health or addictive disease concerns. The individual completes the initial intake- legal documents; BHA, Individualized Treatment Plan, Safety Plan and C-SSRS Assessment which are required within the first 3 days of service. Then within 7 days of the individual seeking services, the clinician will be required to complete the initial verified diagnostic assessment , clinical order for services, and initial individual therapy.

Any additional ongoing assessments completed as demanded by individual client needs and/or by service policy. The Diagnostic Assessment process consists of a face-to-face comprehensive clinical assessment with the individual, family member/responsible caregiver(s), and others significant in the individual's life as well as interviews with collateral agencies/treatment providers/relevant individuals. This position includes data gathering and formulation of an Individual Resiliency Plan/Treatment Plan which are all needed before the provision of mental health services to individuals in need of treatment and support. The purpose of the Diagnostic Assessment process is to perform a formalized assessment in order to determine the individual's problems, strengths, needs, abilities, and preferences, to develop a social (extent of natural support and community integration) and medical history, to determine the functional level and degree of ability versus disability, and to develop or review collateral assessment information. The information gathered should support the determination of a differential diagnosis and assist in screening for/ruling out potential co-occurring disorders. This position requires a broad knowledge of therapeutic processes and methodologies.
Duties and Responsibilities :
1) Receives referrals of clients in need of mental health services. 2) Conduct face-to-face assessments in office and out of the office through telehealth. 3) Ensures the orientation - intake packet is completed thoroughly. 4) Completes and submits the Behavioral Health Rating Scales, Diagnostic Assessment, Safety Plan, Treatment Plan, C-SSRS, and Authorization forms in the electronic Medical System. 5) Completes documentation (Assessment/Treatment/Reauthorization/Counseling Session BIRP note) in electronic medical record system. 6) Familiar with codes, able to identify correct code usage, and the content of note matches units/codes being utilized. 7) Assists the individual with the development of the Individualized Resiliency Plan (IRP)/Treatment Plan. 8) Submits the IRP, and Authorization forms. 9) Completes Individualized Resiliency Plans / Treatment Plans. 10) Participates in various staff meetings and in-services and communicates with Intake Staff. 11) Meet with the Clinical Director weekly to update him/her on any clinical and programmatic issues 12) Contact referring agency Probation Officer or Caseworker once a referral is received and maintain collaboration with referring party. 13) Attend mandatory staff meetings weekly 14) Provides counseling, therapy, and/or psychotherapy to consumers and families on an as-needed basis. 15) Input initial assessment/treatment plan and ongoing goals into Sharenote. 16) Follow guidelines, policies, and procedures of the Assessor Policy 17) Other duties as assigned.
Minimum Qualifications:
Must have a Master's degree in social work, clinical psychology, counseling, or a closely related field and be fully licensed. Experience completing assessments and Individual Resiliency Plans preferred. Demonstrated knowledge of mental health disorders and treatment methods. Ability to provide skilled treatment to clients with social and behavioral problems. Ability to establish and maintain working relationships with consumers, families, service providers, and colleagues. Knowledge of clinical operations and procedures, Medicaid services, commercial insurance payors, and community mental health resources. Advanced knowledge of clinical social work and mental health counseling principles, methods, procedures, and standards. Strong written and verbal communication skills. Exceptional verbal and written communication skills.
