In-Take Coordinator / Patient Care Specialist
I. DEFINITION A
Professional Staff member responsible for the provision of patient intake and care coordination.
II. QUALIFICATIONS A.
Educational 1. Minimum of a HS diploma, College degree preferred B. Professional Experience 1. Minimum of 2 years as a staff preferred. Will train depending on position opening. C. Personal Qualifications 1. Possesses physical and mental integrity with a sense of moral and social responsibility to self and to others. 2. Warm "people oriented" Personality with focus on customer service 3. Establishes and maintains good interpersonal relationships. 4. Each staff member will demonstrate a positive attitude and a team spirit by interactions with co-workers and physicians. 5. Must be able to work in a team environment, yet be able to work and make decisions independently when necessary. 6. Must possess good verbal communication and documentation skills. 7. Must have good organizational skills with the ability to multi-task when necessary.
IV. FUNCTIONS
Each staff member will be able to function in their area of the facility. They will utilize proper and appropriate communication skills and techniques to meet the patient's needs and provide Total Patient-Centered Care. Primary Functions A. Answering telephone calls from referral sources, patients, and other business related entities and transferring messages or calls to the proper parties. B. Take patient referrals from referral sources through, telephone or other referral sources. C. Ensure proper medical documentation is present in chart before visit IE MRI, medical records. D. Call patients to remind them of their appointments and to bring any co-payments of deductibles owed. E. Create schedules for next day schedule F. Ensure patient "flow" from waiting room, to consult /Treatment room, to Discharge room when necessary.•Greet the patient.•Inform patient of all necessary paper work

forms, Narc Agreement, etc.•Collect proper co-pays or amounts owed from patients G. Take medication refill requests and phone calls to distribute them to the proper providers. H. Refill or call in medications as ordered by the physician or nurse practitioner / PA. I. Assist in making proper patient referrals IE MRI or another specialist J. Assist providers with procedure authorizations when necessary. Other Clinic Functions A. Ensure patients are admitted efficiently, courteously, and in a timely manner. Responsible for answering the telephone courteously and appropriately. B. Acquire all necessary insurance information and medical information for chart and billing purposes. Review insurance verification, information with patient and family. C. Schedules all procedures and follow-up visits when appropriate. D. Go over all necessary documentation with patient to ensure patient has understanding of rights and responsibilities. E. Posts charges and collects on patient accounts when necessary (IE patient co-pays)
V. GENERAL RESPONSIBILITIES A.
Maintain a clean, comfortable, private, and professional environment at our facility at all times.•Follow all HIPPA policies and procedures to ensure patient confidentiality. B. Restock supplies in all consult and treatment rooms when necessary as dictated on a daily basis. C. Order medical supplies when necessary. D. Answer phones when necessary. E. Schedule patients when necessary. F. Assist in filing medical records when necessary. G. Assist with verifying insurance information and getting insurance authorizations for office visits and procedures. H. Assist in making I. Assist with filing when deemed necessary. J. Maintain Universal Precautions at all times while working in the facility. K. Work on Quality Improvement issues that pertain to certain areas; problem solve issues to maintain the quality of total patient care. L. Work as a team player, maintain company goals, communicate problems, issues, and etc... to administrator and/or providers when needed. M. Attend all staff meetings, mandatory in-services as required by administration. N. Maintain a good working relationship with peers, co-workers, physicians, and patients. O. Personnel must be able to lift object up to 30 lbs without assistance.
Maintain proper body mechanics in:
1. Lifting patient to and from table, wheelchair, or chair with assistance when deemed necessary while accompanying patients to specific areas of patient care. 2. Moving, unpacking, and lifting supplies and equipment with assistance not to exceed over 30 lbs. 3. If a lifting over 30lbs is required (IE patient transport or supplies) you will find proper assistance to complete the task. P. Perform other duties when deemed necessary by the Practice.
