Job Description

Job Summary We are seeking a compassionate and patient-focused Medical Provider (MD or Nurse Practitioner) to deliver high-quality healthcare services in patients' homes. This role offers the opportunity to build meaningful relationships with patients while providing personalized medical care in a comfortable home setting. The ideal candidate is clinically skilled, adaptable, and committed to improving patient outcomes through comprehensive assessment, treatment, education, and care coordination. Responsibilities Conduct patient evaluations, physical assessments, and review medical histories. Develop and implement individualized treatment and care plans. Diagnose, treat, and monitor acute and chronic health conditions. Prescribe medications and treatments in accordance with applicable regulations and clinical guidelines. Document patient encounters accurately using electronic medical records (EMR/EHR). Collaborate with caregivers, family members, and healthcare professionals to ensure continuity of care. Provide patient and caregiver education regarding medications, disease management, preventive care, and healthy lifestyle practices. Order and review diagnostic tests as clinically appropriate. Participate in follow-up care and ongoing patient monitoring. Utilize telehealth services when appropriate to support patient care. Qualifications Active and unrestricted MD or Nurse Practitioner license in the applicable state. Current certifications and credentials required for practice. Experience in primary care, home-based care, family medicine, geriatrics, internal medicine, or related healthcare settings preferred. Strong clinical assessment and decision-making skills. Experience using EMR/EHR systems for documentation. Knowledge of patient privacy and confidentiality standards. Excellent communication, interpersonal, and organizational skills. Ability to work independently while collaborating effectively with a multidisciplinary care team. Valid driver's license and reliable transportation, if required.
