JOB SUMMARY
As an important team member of Saint Francis, the MyCare Connect Coordinator utilizes critical thinking; the position evaluates care journey needs, aligns patients with appropriate care, and facilitates timely and coordinated patient access. Additionally, the position triages patient friction points, and serves as a liaison between patients, providers, and interdisciplinary teams to ensure coordinated, efficient, and high-quality care delivery.JOB DETAILS AND REQUIREMENTS
