1:8 Weeks. Average week 3-5 total callsPatient panel max of 400Average patients per day: 10-1310k Square Ft. Center with Labs, EKG and Echo'sPhysicians each supported by own scheduler, MA, referral specialistPatient Care team: Nurse, Case Manager, Social Worker & Outreach Specialist as well as a social community health coordinatorEach clinic/center invites patients for cooking classes, yoga, silver sneakers-mall walking exercise community for seniorsPACKAGESalary Range $250k for a resident. PCP with Value-Based Care Experience $280k/ Experienced PCP without Value-Based Care experience $265k-$270kSign on: Up to $15k.
