Patient Access Facilitator The Patient Access Facilitator manages inpatient, observation, and swing bed care, ensuring smooth discharge planning and patient-centered communication with providers, families, and caregivers. This role supports hospital leadership, oversees staff scheduling, conducts utilization reviews, and serves as the facility's HIPAA Officer. Strong clinical experience, adaptability, and teamwork are essential. Prior scheduling and EHR experience preferred.
Position Description:
Responsible for carrying out specific tasks associated with coordinating all aspects of care for inpatient hospital, observation, and skill swing beds. Must work side by side with providers to meet patient needs. Will act as facility discharge planner; communicating patient needs to provider, caregivers, and family in a friendly professional manner with a focus on the patient. Participates/facilitates in creating & maintaining professional, patient centered teamwork approach at the hospital. Responsible for utilization review.

Officer Assists Director of Nursing and Assistant Director of Nursing with hospital projects. Scheduling coordination for
RN, LPN, and Aides Requirements:
3 years of clinical experience MSW, LCSW, LPN, or Community Health Worker. Must be highly motivated and able to adapt to a variety of situations as they occur. Ability to communicate patient needs in an efficient and effective manner. Able to prioritize and multitask. Ability to conduct yourself at all times in a manner consistent with and that promotes MCH's core goals of patient care and teamwork. Able to adapt to changing roles. Creative and innovative Be a role model of outstanding teamwork and leadership with the ability to communicate with all team members with respect, dignity, compassion, and collaboration. Prior scheduling experience preferred. Knowledge and experience with Electronic Health records is preferred. Compensation commensurate with experience & expertise.
