Where You Will Work :
This position is for a Medicare Wellness Nurse who will support multiple (typically 1-4, depending on Medicare patient volume) primary care clinics in the Concord area. The Medicare Wellness Visit (MWV) focuses on preventive care, helping patients close care gaps and meet quality measures while maintaining strong engagement with their primary care team. In this role, the RN will obtain vital signs, administer Medicare-covered vaccines, complete a comprehensive health risk assessment, and develop a personalized prevention plan for each patient.We're Looking For:
Completion of an accredited or approved program in nursing Registered Nurse license issued by the state in which the teammate practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association Adult care nursing experience (any length) Minimum of 5 years of nursing experience (excluding schooling/practicum) Bachelor of Science in Nursing (BSN) preferred Board certification in Gerontology or Ambulatory Nursing (required within 2 years of hire)What You'll Do:
