Are you a compassionate, motivated person who loves to help people? Join us at DMC Primary Care as a Patient Care Liaison ! As a Patient Care Liaison (PCL), you will be an important member of the health care delivery team, focusing on delivering high-quality administrative tasks in close collaboration with our clinical care teams. Experience in a medical office is preferred, but a positive, can-do attitude, a willingness to learn, and a genuine desire to help patients is even more important to us. The successful candidate must be able to multi-task under pressure and always deliver patient service with the highest level of professionalism. The successful candidate for this full-time position will split their time between our Raymond and Windham clinics, Monday - Friday from 8:00 am to 5:00 pm. Following are some of the duties of a PCL (this is not an all-inclusive list): Monitor and sort EMR (Electronic Medical Record) messages to assure timely completion of messages in accordance with DMC Policies & Procedures. Relay results to patients including labs, imaging, and other testing. Helps patients obtain referrals, and refills as clinically indicated. Enter imaging, and lab orders under the direction of a licensed medical provider, following established DMC guidelines. Coordinate the scheduling of appointments at DMC and also with specialists and outside facilities. Communicate with patients via all patient communication platforms, according to patient preference. Document all patient communication appropriately using professional etiquette and technique. Take an active role in gaining and maintaining proficiency in Allscripts EMR system, clinic flow, and documentation standards. Possess high-level organizational skills and ability to multi-task and work under pressure, while presenting a calm, professional manner and positive attitude.
