Full Job Description Position:
Intake Representative, Full-Time Location:
Brooklyn Office (1989 Coney Island Avenue) Bilingual in Spanish or Russian preferred• HCS Home Care is seeking an Intake Representative that will be responsible for managing all aspects of the patient intake process. This includes handling medical records, data entry, maintaining patient records, coordinating telephone calls, and managing insurance verification and authorization processes. Review and process referrals from hospitals, physicians, patients, or families to determine eligibility and need for home health services, ensuring compliance with federal, state, and agency regulations. Coordinate with healthcare professionals to obtain patient medical history, documentation, and insurance information necessary for admission and billing purposes. Schedule initial home visits for the clinical team, ensuring timely and efficient start of care for patients. Educate patients and families about home health services, care plans, and the roles of various healthcare professionals involved in their care. Facilitate communication between patients, families, and the home health care team to ensure coordination of services and adherence to the care plan. Maintain accurate and comprehensive patient records in the agency's electronic health record (EHR) system, including documentation of all interactions, referrals, and coordination activities. Assist in the development and implementation of intake policies and procedures to improve efficiency and effectiveness of service delivery. Perform quality assurance reviews of intake documents and patient files to ensure accuracy, completeness, and compliance with regulatory standards.
