We are hiring for an experienced nurse (RN) to work in our adult day program 2 days per week providing care for patients serviced by OPWDD.

Per HR Description:
  • We are seeking a Registered Nurse for a Day Habilitation Program.
  • This role involves supporting individuals with developmental disabilities in a structured residential setting.
  • The position is part-time and located onsite in Brooklyn, NY, requiring close collaboration with interdisciplinary teams to manage the medical needs of residents.
Requirements:
  • Conduct comprehensive nursing assessments
  • Develop individualized care plans
  • Monitor and evaluate medical needs
  • Maintain clinical documentation
  • Coordinate care with healthcare professionals
  • Supervise and support AMAP staff
  • Participate in treatment planning meetings
Qualifications:
  • Active NYS RN license
  • Experience in residential or OPWDD settings preferred
  • Experience with adult populations and developmental disabilities preferred
  • Strong documentation skills
  • Effective care coordination skills
