Job Description
Join our team as an SCN Intake CoordinaNavigator and play a vital role in supporting individuals and families within the Muslim community by helping them access the healthcare, social, and community resources they need to improve their health and well-being. As the first point of contact, you will guide members through the enrollment process, assess eligibility, identify unmet social and health needs, and connect individuals with appropriate services and supports. The ideal candidate is compassionate, culturally aware, and comfortable working with members of the Muslim community. Bilingual candidates who speak languages commonly used within the community are strongly encouraged to apply. Key ResponsibilitiesScreening & Intake Conduct eligibility screenings and assessments for SCN services. Collect demographic, medical, insurance, and social needs information. Identify barriers to care, including housing instability, food insecurity, transportation challenges, unemployment, behavioral health concerns, and chronic health conditions. Build trust and communicate effectively with members and their families while being sensitive to cultural and community needs. Enrollment & Navigation Guide eligible members through the enrollment process. Connect members with available benefits and services, including transportation, meal programs, health education, personal support services, and other community resources. Provide culturally appropriate support to ensure members have access to the care and resources they need. Ensure a smooth transition into ongoing care and support programs. Documentation & Compliance Maintain accurate and timely records in accordance with SCN and Department of Health requirements. Track incomplete screenings and follow up to ensure completion. Protect member confidentiality and maintain compliance with all applicable regulations. Member Support & Coordination Assist with the development of individualized service plans. Collaborate with care managers, healthcare providers, hospitals, behavioral health programs, and community organizations. Monitor member engagement, service utilization, and progress toward goals. Conduct outreach and follow-up when members encounter new barriers or disengage from services. Qualifications Strong communication, interviewing, and relationship-building skills. Ability to conduct assessments and gather detailed information with accuracy and compassion. Cultural sensitivity and ability to work effectively with members of the Muslim community. Knowledge of healthcare, community resources, and social service systems preferred. Excellent organizational and time management skills.
