Job Description

We are seeking a compassionate, organized, and proactive Intake Coordinator to be the first point of contact for prospective clients and referral sources. In this role, you will guide new clients through the intake process, assess their needs, and connect them with the services that best support their goals. Responsibilities Manage incoming referrals and client inquiries. Conduct intake assessments and gather required documentation. Educate clients and families on available services and eligibility requirements. Coordinate with internal teams to ensure appropriate service placement. Verify information, maintain accurate records, and track intake progress. Provide exceptional customer service and follow-up throughout the onboarding process. Qualifications Strong communication and relationship-building skills. Excellent organizational and multitasking abilities. Experience in healthcare, social services, customer service, or intake coordination preferred. Ability to work efficiently in a fast-paced, client-focused environment. IND123
