Job Description Summary:

Monday- Friday, No Weekends & No Holidays
Hours of Operation:
7:00am-5:00pm, schedule depends on business needs Join our primary care team as a Licensed Practical Nurse (LPN) and make a meaningful impact on the health and well-being of adult patients in a collaborative, patient-centered practice. This full-time position offers a consistent Monday-Friday schedule with no evenings, weekends, or holidays, giving you the opportunity to enjoy an excellent work-life balance while building lasting relationships with patients and your care team. In this role, you'll support providers by managing clinical messages and refill requests, triaging patient calls, coordinating referrals, and responding to MyChart communications. You'll also perform direct patient care responsibilities, including obtaining vital signs, administering injections, completing EKGs, and assisting with preventative care and annual wellness visits. We're looking for an experienced LPN with 2-3 years of clinical experience who is confident working independently, communicates effectively, and thrives in a fast-paced environment where teamwork and initiative are essential. This position provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. The LPN delegates nursing tasks to unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act. This position serves as a resource for other associates. May communicate instructions, do minor scheduling, and inspect work. Does not hire, fire, discipline, promote, demote, transfer, or appraise, but may be consulted by higher management in these areas. This job has no supervisory responsibilities.Responsibilities And Duties:
- 25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual.
Minimum Qualifications:
- High School or GED (Required)BLS
- Basic Life Support
- American Heart Association, LPN
- Licensed Practical Nurse
- National Council of State Boards of Nursing
Additional Job Description:

- Must hold a current license from the State of Ohio. successfully completed Ohio Board of Nursing approved medication course. Basic Life support Provider
