Full-Time Patient Wellness Coordinator Opportunity split between Berlin, PA! Join our Passionate Rehab Team at Meadow View Nursing Center! - Berlin, PA Continuum Therapy Partners is excited to offer a Part-Time opportunity for a Patient Wellness Coordinator at Meadow View Nursing Center in Berlin, PA a supportive, collaborative skilled nursing facility where your expertise makes a daily impact. Duties and Responsibilities Following verification of insurance eligibility, benefits, and coverage for therapy services by the facility"s BOM, staff member will obtain and track prior authorizations (PT, OT, ST) Assists with resident treatments assigned by the licensed therapist. (Only unskilled and non-billable assistance can be provided.) Prepares residents for treatment as instructed by supervising therapist. Transports residents to and from the Rehabilitation Department for scheduled treatments in a timely and safe manner as requested. Assists with and participates in quality assurance programs as required. Adheres to established confidentiality standards per Clerical Authorization access as defined in the HIPAA policy and procedure manual. Follows management direction. Performs the operation of rehabilitation equipment for which training has been received and competency demonstrated. Communicates orally or in writing with clarity and accuracy. Projects a positive and professional image at all times. Performs department secretarial functions as directed. Maintains appropriate relationships and rapport with coworkers, residents, family members and facility personnel. Confers formally or informally with other team members in coordinating the administrative functions of patient care. Attends required meetings and in-services. Maintains department in neat, clean, orderly and safe manner. Assists with setting up treatment areas by:
Qualifications Education:
High school diploma or GED, Leadership experience if preferred Strong organizational skills, communication skills, and attention to detail Ability to manage multiple facilities and deadlines Physical Requirements and Working Conditions Physical requirements for this position are classified as medium under the Department of Labor classification. The employee must be capable of exerting 20 to 50 pounds of force occasionally (less than 1/3 of the time), and/or 10 to 20 pounds of force frequently ( of the time), and/or up to 10 pounds of force constantly (2/3 of the time) to move objects, equipment and/or residents. While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, kneel, bend or crouch; use hands to manipulate tools, equipment or controls; reach with hands and arms, balance, lift and perform medium to maximum transfer assists with residents. The employee is required to have visual and hearing acuity sufficient enough to assess resident safety and ability. Ability to apply principles of rational systems to solve problems and work with a variety or variables and to interpret instruction presented in various formats. Proficient in various software applications including MS Windows, word processing and spreadsheet programs. Ability to clearly and concisely communicate both orally and in writing with all levels of management and staff. Ability to understand instruction and execute assigned tasks.
