Job Summary We are seeking a dedicated and energetic DDA Intake Coordinator to join our dynamic healthcare team. In this vital role, you will serve as the first point of contact for individuals seeking services, coordinating intake processes with compassion and efficiency. Your expertise will help streamline patient assessments, facilitate access to care, and ensure seamless communication across multidisciplinary teams. The ideal candidate will possess a strong background in hospital settings, with experience in acute care, trauma centers, mental health, and social work, all while demonstrating exceptional organizational skills and a passion for patient-centered service. Duties Conduct comprehensive intake assessments for individuals seeking services, including medical history, psychosocial factors, and current needs Coordinate with hospital departments such as emergency medicine, ICU, pediatrics, and behavioral health to facilitate patient evaluations and service planning Manage electronic health records (EHR) and ensure compliance with HIPAA regulations to protect patient confidentiality Collaborate with community resources and social services to support discharge planning and post-acute care arrangements Prioritize tasks effectively to handle high-volume intake workflows while maintaining accuracy and attention to detail Communicate clearly with patients and their families to explain procedures, gather information, and provide guidance on available services Support clinical case management by documenting assessments, updating medical records, and coordinating multidisciplinary interventions Experience Previous hospital experience in a Level I or Level II trauma center setting is highly preferred Demonstrated expertise in acute care environments such as ICU, emergency medicine, or inpatient units Experience working with children, individuals with disabilities, mental health conditions, or substance use disorders Familiarity with medical terminology, diagnostic evaluation processes, and clinical case management practices Proficiency in electronic health records (EHR) systems and healthcare data management tools Knowledge of community resource coordination within health services social work frameworks Strong background in patient assessment, discharge planning, utilization management, and behavioral health support Excellent communication skills for working with diverse populations including families and multidisciplinary teams Ability to prioritize tasks efficiently in fast-paced hospital or outpatient settings while maintaining high-quality patient service Join us in making a meaningful difference by ensuring patients receive timely access to the care they need.
