  • Private Pay Home Care Modivcare Personal Care Services is seeking an Intake Coordinator with direct home care experience and a strong client-service mindset to support prospective private-pay clients and families from initial inquiry through successful start of care.
Pay:
$24.68

50/hour, depending on experience
Schedule:
Monday-Friday, 8:30 AM-5:00

  • and Long-Term Disability; Accident, Critical Illness & Hospital Indemnity Insurance What You'll Do Manage new private-pay inquiries and referrals, providing prompt, professional, and high-touch support to prospective clients and families.
Conduct intake conversations to understand care needs, schedules, preferences, location, and desired start-of-care dates. Educate clients and families on home care services, rates, service expectations, and the private-pay onboarding process. Maintain consistent follow-up with prospective clients, answer questions, address barriers, and help move inquiries toward successful onboarding. Coordinate assessments, required documentation, caregiver matching, staffing, and start-of-care activities with internal teams. Maintain accurate intake records and track prospective clients from initial inquiry through successful start of care. What You'll Need 1+ years of direct home care experience in intake and/or care coordination required. Private-pay home care experience strongly preferred. Home care scheduling, staffing, or caregiver-matching experience preferred. Experience providing high-touch or white-glove service to clients and families preferred. Experience handling caregiver coverage, schedule changes, and start-of-care coordination preferred. Experience with HHAeXchange, payer portals, or similar home care systems preferred. Bilingual English/Spanish strongly preferred. Strong communication, organization, follow-through, and computer skills.
