Job Summary:
Patient Care Associate (PCA) identify patients eligible for remote monitoring, enroll them in the program, monitor their data, and coordinate their care with their physicians.Responsibilities:
Patient Onboarding- Identify patients eligible for remote monitoring. Enroll patients at their Physician's practices, at patient's home, or remotely. Assist with device setup, and train patients on the purpose, benefits, and usage of RPM. Device Management
- Ensure that devices are properly configured, calibrated, and maintained. Coordinate device replacements or upgrades, as needed. Email, phone and/or live chat with technical team for issues, troubleshooting, and problem resolution, with patient devices. Data Management
- Oversee the collection, transmission, and storage of patient data. Monitoring and Analysis
- Monitor patient data, analyze trends, identify abnormalities or significant changes in patient health status, and escalate issues to appropriate healthcare providers for intervention. Patient Engagement
- Provide ongoing support, encouragement, and communicate with patients and/or caretakers to promote adherence to RPM protocols and healthy behaviors. Documentation
- Provide routine, timely, and complete documentation into RPM platform and Physicians' electronic health records. Collaboration
- Liaison and collaborate with physicians and their staff, to coordinate patient care and optimize the use of RPM within the broader healthcare ecosystem.

Must have reliable mobile phone, laptop, internet connection, and transportation. In-person visits to physician practices and patient homesEducation :
