The Role :
The Care Specialist supports the Social Recreation service line within Disability Services, helping individuals successfully access and maintain person-centered services. This role manages a caseload from referral through ongoing service delivery, conducts assessments and develops Individual Service Plans (ISPs), supports Direct Support Professionals (DSPs), and coordinates with families, Regional Centers, and internal teams. The Care Specialist helps ensure services are delivered effectively, goals are monitored, and program requirements remain compliant. Primary Responsibilities Conduct assessments and develop Individual Service Plans (ISPs), person-centered goals, progress reviews, and required six-month and annual reporting for assigned individuals. Manage a caseload of individuals and families from referral through ongoing services, providing timely communication, follow-up, and support throughout the service lifecycle. Facilitate weekly training and provide ongoing guidance to Direct Support Professionals (DSPs) to promote community inclusion, socialization skills, and progress toward individual goals. Coordinate with Regional Center staff, families, DSPs, and internal teams to support service delivery, resolve concerns, manage authorizations, and address potential risks. Maintain accurate records and performance metrics while ensuring services and documentation comply with Regional Center requirements, Title 17, HIPAA, and applicable state and federal requirements. This is a hybrid position, coming into the El Segundo office 1x per month.
Work Schedule:
Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs, employees may be required to work any scheduled shift within these operating hours. What You Bring to the

Qualifications Bachelor's degree preferred. Minimum 3 years of experience in person-centered practices, developing care plans, or a related service-delivery environment strongly preferred. Certification or experience in person-centered planning and thinking preferred. Experience supporting individuals with intellectual and developmental disabilities or working within social recreation or related services preferred. Bilingual Spanish preferred.
