Apply Now Job Description Responsibilities:
Works closely with the MD, Director of DH, AD of PAT, staff and ancillary staff by assisting in the coordination of the medical, emotional, physical, mental and spiritual needs for outpatients. Works closely with the patient, patient's family and/or healthcare agent, physician, nurses and ancillary staff to assure a continuum of care for patients. Utilizes knowledge of human growth and development to assess, interpret data, and initiate and evaluate a plan of care appropriate to the pediatric, adolescent, adult and geriatric patient. Is knowledgeable about Blood Management by training and/or experience. Assures that the patients' rights and wishes to accept or decline blood transfusions are recognized, and appropriate Blood Management alternatives and techniques are employed. Serves as an Advanced Consultant to patients, families and staff in the delivery of care to patients choosing to refuse consent for blood transfusions for religious and/or personal reasons. Provides support and educational materials specific to refusal to accept blood and assists patients in completing any necessary forms. Works closely with the appropriate staff and ancillary departments to facilitate any needed appointments/referrals and to ensure effective communication and a smooth process throughout the surgical journey. Ensures that patients' rights and privacy are maintained to a very high degree. Uses appropriate documentation to ensure collaboration of care among staff. Be active in promoting, planning, developing and implementing any growth initiatives regarding position. Assist in development and implementation of policies and processes as needed. Provide and maintain data on patients seen for review and seek opportunities for improvement. Round on patients in Surgical waiting area, DH, HA, and Endo to provide updates on progress. Maintain communication with staff, physicians and family to facilitate correct and up to date information for patients as well as family members throughout the surgical journey.
