JOB SUMMARY

Primarily responsible for receiving and evaluating referrals for clinical eligibility of Home Health services. Responsible for entering all required intake information into EMR and communicating information to scheduling and the clinician for patient admission to services.

JOB REQUIREMENTS

LPN Required High School Diploma or equivalent Preferred 2 Years Medical Surgical Nursing Preferred 2 Years Home Health, Community Health or Discharge Planning Preferred
