Conducts pre-visit chart reviews of patients, identifying needed preventive health maintenance and immunizations. Completes daily chart preparation prior to patient appointments. Performs medical record reviews, data collection, chart abstractions, reporting, and analysis. Coordinates and performs onsite medical record reviews. Identifies patients overdue for visits, labs, or referrals, and arranges follow-up services to assist with care gap closure. Assists in developing and implementing frontline practice strategies to address and close care gaps and increase compliance with preventive care recommendations. Researches best practices, national and regional benchmarks, and industry standards of care. Collaborates with leadership to produce reports, and evaluates utilization and quality performance indicators. Monitors for adverse trends and recommends modifications and corrective actions. Participates in relevant committees and work groups, making presentations and preparing reports, data, or other materials for presentations. Other duties as assigned.Knowledge, Skills, and Abilities:

- Previous experience with electronic medical records [Preferred]
Education:
- Associate [Preferred]
- High School Grad or Equiv [Required]
Field of Study:
- in a health related field
Work Experience:
- 1+ experience in a healthcare environment [Preferred]
