Apply for our Physical Therapist (PT) and Physical Therapist Assistant (PTA) job opening today! Duties Conduct comprehensive assessments of patients to determine their physical therapy needs. Develop and implement personalized treatment plans based on individual patient goals. Utilize various therapeutic techniques including trigger point therapy to alleviate pain and improve mobility. Educate patients and their families on treatment plans, exercises, and rehabilitation techniques. Collaborate with multidisciplinary teams to ensure cohesive patient care across inpatient, outpatient, and acute care settings. Maintain accurate documentation of patient progress and treatment outcomes in accordance with medical terminology standards. Participate in discharge planning to facilitate smooth transitions for patients leaving skilled nursing facilities or acute care environments. Stay current with advancements in physiotherapy practices and integrate new techniques into patient care. Qualifications Valid state licensure as a Physical Therapist (PT) or Physical Therapist Assistant (PTA). Knowledge of anatomy and physiology relevant to physical therapy practices. Experience in various settings such as geriatrics, pediatrics, orthopedics, and skilled nursing facilities is preferred. Familiarity with discharge planning processes and care plan development for diverse patient populations. Strong communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work collaboratively within a team-oriented environment while maintaining a focus on individual patient needs. Previous experience in home health or outpatient clinic settings is a plus.
