Choice Health at Home is seeking a Home Health Intake Coordinator who thrives in a fast-moving environment and understands that intake is much more than paperwork. You are the front door to home health care. Every referral represents a patient waiting to receive care. Your job is to make sure referrals move quickly, accurately, and smoothly from "We have a patient" to "We're ready to provide care." What You'll Own As our Home Health Intake Coordinator , you'll help drive the referral and admission process from beginning to end. As a Intake Referral Coordinator, you will: Receives and appropriately documents new referrals in a timely manner. Verifies each patient's payor source and maintains and tracks insurance authorizations and initial certifications. Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group. Provides effective communication to patients, staff members, other health care professionals and referral sources. Responds to Agency and patient needs in a professional and creative manner. Demonstrates commitment, professional growth and competency. Participates in Agency-sponsored in-service training. Participates in the Agency's QAPI program. Promotes the Agency philosophy and administrative policies to ensure quality of care. Requirements Minimum one year of experience in referral intake is preferred Experience with third party payers, Medicare and Medicaid highly desirable Medical Field background preferred, including familiarity with medical terminology Excellent computer and customer service skills LVN/LPN preferred Valid Driver's License Valid Auto Insurance HomeCare HomeBase experience preferred Benefits and Perks Medical, Dental, Vision Insurance 401k You're eligible after 3 months of service/The plan is 100% fully vested immediately /Choice Health At Home contributes 100% of the first 3% you contribute each pay period Health Savings Account Life Insurance Short & Long Term Disability Insurance Paid Time Off and Paid Holidays
