Intake Coordinator - Home Health Full-Time | Nayar Health Care Nayar Health Care is seeking a dependable, organized, and motivated Intake Coordinator to join our growing Home Health team. This position plays an important role in ensuring new referrals are processed quickly and accurately while providing excellent communication with referral sources, patients, families, and our clinical team. What You'll Do Receive and process new Home Health referrals Verify patient demographics, insurance, and eligibility information Review referrals for required documentation and follow up on missing information Enter and maintain accurate patient information in the EMR Communicate with hospitals, physician offices, facilities, and other referral sources Coordinate with clinical leadership regarding acceptance and scheduling of new patients Track pending referrals and follow through until admission or non-admit Maintain accurate documentation and ensure HIPAA compliance Assist with additional intake and administrative responsibilities as needed What We're Looking For Previous Home Health, healthcare intake, scheduling, or medical office experience preferred Strong organizational skills and attention to detail Excellent communication and customer service skills Ability to multitask and manage a high-volume workload Comfortable working with EMR systems and insurance verification Dependable, responsive, and able to work well with a team Why Join Nayar Health Care?
