Job Description
Primary Function/General Purpose of Position The Intake Coordinator serves as the primary point of entry for individuals seeking access to programs and services. This role focuses on intake, assessment, and referral management to ensure timely and appropriate connection to internal programs and external community resources. The Coordinator provides a streamlined process for clients/residents, programs, partners, and community members, ensuring equitable access and efficient navigation of available services. Essential Job Functions Serve as the primary point of contact for individuals seeking access to programs and services. Conduct intake interviews to collect demographic, health, and social needs information and assess program eligibility. Receive, process, and coordinate referrals from internal programs, external partners, and community members. Manage referral workflows and follow-up activities to ensure timely connection to appropriate services and resources. Provide clients with clear guidance on available programs, next steps, and barriers to accessing services. Maintain accurate and up-to-date documentation, records, and data within designated systems to support compliance and program evaluation. Generate and monitor reports related to intake activity, referral outcomes, and identified service gaps. Maintain effective working relationships with internal teams, community organizations, and external partners to strengthen referral networks and service coordination. Support outreach and community engagement efforts to increase awareness of intake processes and available services.
