Sign-On Compensation:
$750 Prorated by Hire Date I.
Qualifications:
1. Bachelors/Masters Degree in Speech-Language Pathology 2. Current licensure from the Arizona Department of Health Services (A.D.H.S.) 3. Must possess a valid Arizona IVP Fingerprint Clearance Card 4. Must be able to pass a background check upon employment II.
Duties and Responsibilities:
1. Screen and evaluate appropriately referred students within the State and Federal requirements. 2. Attend and actively participate in all appropriate Child Study Team (C.S.T.), Multidisciplinary Evaluation Team (M.E.T.), and Individualized Education Program (I.E.P.) team meetings. 3. Develop speech-language related IEP goals, objectives, and/or benchmarks, including any appropriate accommodations for qualified district students as assigned. 4. Complete all State and Federally required paperwork within required timelines. 5. Provide individual and group speech-language services to students as described in the IEP. 6. Prepare quarterly, or more frequent if needed, progress reports on each assigned student to include in the quarterly grade reporting process. 7. Complete all
MEDICAID
reports and requirements in a timely manner. 8. Participate in District training as appropriate. 9. Perform other related duties as may be assigned by the Director of Special Services.
