You'll provide individualized, evidence-based treatment plans and collaborate closely with our interdisciplinary team to deliver exceptional outcomes. Primary Responsibilities Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders. Develop and implement individualized treatment plans. Perform bedside and clinical swallowing evaluations. Collaborate with physicians, nursing, occupational therapy, physical therapy, dietary services, social services, and families. Maintain accurate and timely documentation in accordance with Medicare, state, federal, and facility requirements. Participate in interdisciplinary team meetings and care planning. Educate residents, caregivers, and staff regarding communication and swallowing strategies. Promote the highest level of resident independence and quality of life.
