You'll play a vital role in helping patients regain communication, swallowing, and cognitive function in a collaborative, supportive skilled nursing environment. What You'll Do Evaluate patients within 24 hours (or 1 business day) of physician referral Develop and implement individualized treatment plans Provide therapy per physician orders Monitor, document, and adjust patient progress as needed Collaborate with physicians, nursing staff, and interdisciplinary team members Participate in care conferences, rehab meetings, and discharge planning Educate patients, families, and caregivers for continued care Provide in-services and training for facility staff Maintain accurate documentation using EMR systems Qualifications Master's or Doctorate degree in Speech-Language Pathology Active California SLP license (required) Strong clinical, communication, and documentation skills Skilled nursing or post-acute experience preferred, but not required Team-oriented with the ability to thrive in a fast-paced environment Certifications & Requirements Current SLP license in good standing with the state Ongoing continuing education per state requirements Familiarity with EMR systems Why Join Citrus Heights Post Acute?
