Come join us! Grace Community Home Health is a Christian/Faith-Based Agency seeking 2 per-diem Speech Language Pathologists (SLP). Speech Language Pathologist Home Health Position Summary Full independence and schedule flexibility Independently evaluate and conduct follow-up visits as a Speech Therapist Provide Speech Therapy services to clients in their home Document patient assessment and visit details in EHR and notify Clinical Supervisor of findings Communicate care and updates with the care team Submit documentation of Evaluation and Follow up visits according to the company policies Evaluate and create plan of care for patients independently Qualifications Friendly, cheerful, and excellent customer service/bedside manners 1 year as a Speech Therapist in an acute or rehab facility Active California Speech Therapy License We offer competitive pay and career advancement opportunities. Pay is per visit. ~$128k-148k estimated annualized salary.
