JOB SUMMARY
The Speech Language Pathologist (SLP) in Home Health is responsible for assessing and evaluating patients' care needs related to speech and language disorders, as well as providing functional training in communication, swallowing, and cognitive impairments. Services are delivered within the scope of practice defined by state laws governing speech therapy, in alignment with the care plan, and in collaboration with other healthcare team members. The therapist evaluates the patient's functional abilities through diagnostic and prognostic assessments, supporting the physician in creating the therapy plan. Treatment focuses on helping patients communicate effectively, tailored to their condition, and following established standards of practice.
RESPONSIBILITIES & DUTIES
: Conduct thorough assessments of patients' speech, language, communication, and swallowing abilities to identify deficits and develop individualized therapy plans. Design and implement targeted therapy programs to address specific disorders, including speech articulation, language comprehension and expression, cognitive-communication skills, and dysphagia (swallowing difficulties). Educate patients and their families on therapy techniques, home exercises, and strategies to support and enhance communication and swallowing functions. Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members. Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated and holistic approach to patient care. Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards. Participate in team meetings to contribute to comprehensive care planning and decision-making. Stay current with advancements in speech therapy practices and participate in ongoing professional development. Perform all other duties assigned.

Qualifications/Educational Requirements:
Must be currently licensed in Speech Language Pathologist in the state of California Current CPR certification ( A Heart Association with BLS Hands-on return Demo
REQUIRED
) Holds a certificate of clinical competence and licensure granted by the
American Speech and Hearing Association Additional Requirements:
TB test Proof of eligibility to work in the US Background check and drug screen Reliable transportation
