Where You'll Work Founded in 1928, Dignity Health - St. Bernardine Medical Center is a 342-bed, acute care, nonprofit hospital located in San Bernardino, California. One California Job Summary and Responsibilities As our Speech Language Pathologist, you will be a vital clinician, expertly assessing, diagnosing, and treating communication and swallowing disorders to significantly improve patient quality of life. Every day, you will conduct comprehensive evaluations for all referred patients, meticulously implementing individualized treatment plans. You'll contribute clinical expertise to develop/upgrade treatment programs/protocols, and actively shape departmental and organizational goals. To be successful in this role, you will possess advanced clinical knowledge, exceptional diagnostic/therapeutic skills, and a compassionate, patient-centered approach. Appropriately assesses and treats patients of varying diagnoses and disorders requiring Speech /Swallow/Cognitive Therapy, according to established guidelines. Identifies appropriate treatment plan and goals, consistent with physician's referral, patient's diagnosis , abilities and age, using objective measurable data. Comprehensively assesses, treats and progresses patients with swallowing/dysphagia isses, utilizing, but not limited to bedside swallow, MBSS testing and passy muir valve. Communicates professionally and effectively (SBAR format) with the entire health care team (rehab, nursing, physicians, case managers) , to effectively provide the best possible care for the patient. All required Medical record documentation is timely, complete, legible and compliant with all departmental standards and regulatory guidelines. Communicates with Director and supervisor effectively and in a timely manner, regarding any issues, requests or ideas. Completes and forwards all required hospital and department forms, such as timecards, time off requests, projects, without need for reminders. Consistently maintains productivity level at or above department standard and demonstrates excellent time management, including adjusting schedule as needed to complete assignments and using downtime effectively.
