Job Description

Powerback Rehabilitation is seeking a Speech Therapist to join our Healthcare & Medical Services team specializing in post-hospital rehabilitation and recovery care. In this role, you will assess and treat adults with speech, language, cognitive-communication, and swallowing disorders following surgery, injury, or illness. You will design individualized treatment plans, deliver intensive, evidence-based therapy, and collaborate with an interdisciplinary team focused on restoring independence. Responsibilities Evaluate patients' speech, language, cognitive-communication, and swallowing disorders. Develop and implement individualized treatment plans to support recovery goals. Deliver evidence-based speech, language, and swallowing therapies in post-acute settings. Collaborate closely with physicians, nurses, PT/OT, and families on coordinated care plans. Document assessments, progress notes, and outcomes in accordance with regulatory standards. Educate patients and caregivers on home exercises, communication strategies, and safety. Participate in interdisciplinary care conferences and contribute to discharge planning. Use therapeutic technologies and adaptive communication tools to maximize patient outcomes. Monitor, reassess, and adjust treatment plans based on patient progress. Maintain professional licensure, continuing education, and adherence to ethical guidelines.
