A Speech Language Pathologist diagnoses and treats speech, language, and swallowing disorders in patients with a variety of conditions. Helps patients improve their ability to make sounds or use language, or develop non-spoken means of communication. May specialize in working with patients of a certain age group, or treat speech problems resulting from a condition such as a stroke or cleft palate. Works in schools, hospitals and other healthcare facilities and works closely with physicians, psychologists, social workers, and other therapists.

Job Description
The Encore at Boca Raton Rehabilitation and Nursing Center seeks a Speech Language Therapist to provide evaluation and treatment for adults in a skilled nursing and rehab setting. Responsibilities include assessing speech, language, cognition, and swallowing; developing individualized treatment plans; delivering 1:1 and group therapy; documenting progress; collaborating with an interdisciplinary team; educating residents and families; and supporting safe discharge planning. Responsibilities Evaluate residents' speech, language, cognitive-communication, and swallowing functions using standardized and functional assessments. Develop and implement individualized, evidence-based treatment plans for adult and geriatric residents. Provide 1:1 and group therapy to address communication, cognition, voice, and dysphagia needs. Document evaluations, progress notes, and discharge summaries in the EHR in a timely, accurate manner. Collaborate with nursing, rehab, medical, and social work teams on care plans and discharge planning. Educate residents, families, and staff on communication strategies and swallowing safety. Monitor and adjust treatment goals based on resident progress and clinical findings. Ensure compliance with regulatory, payer, and facility documentation standards. Participate in interdisciplinary care conferences and quality improvement initiatives. Support a resident-centered environment that promotes dignity, comfort, and quality of life.
