PRN Speech Therapist

Be one of the first to embrace innovation in home health Patients First Home Care LLC a Medicare certified agency is looking for a Per Diem Speech Therapist to provide in home visits to our patients in the Tampa, FL area. Provide a comprehensive patient examination/evaluation. Complete Oasis assessments timely and accurately. Reassess patient every 30 days and make necessary revisions to the plan of care in conjunction with the physician and documents the patients continual needs and progress. Supervision of Home Health Aides with updates to care plan as needed. Participates in patient care conference, updates, and assists other members of the care team. Participates in; in services, chart audits, quality improvement activities.

Requirements:

Current license to practice Speech Therapy in the State of Florida Graduated from an accredited Speech therapy educational program One year of Speech therapy experience. Preferably 1 year home health experience/ A level II background screening is required for this position https://info.flclearinghouse.com Current Driver's License with current auto insurance CEU on Elderly Abuse, Human Trafficking and Alzheimer's/Dementia TB test results CPR Certification.
