Job Title:
Home Health Speech-Language Pathologist (SLP) - Mableton, GA Heartify In Home Care is seeking a licensed and dependable Speech-Language Pathologist to provide in-home speech therapy services for an adult male client in Mableton, Georgia 30126, flexible schedule 1-2 time weekly. The client has a tracheostomy, feeding tube, and swallowing difficulties. We are seeking a therapist with experience working with medically complex adult patients. Responsibilities Complete a speech therapy evaluation and develop an individualized treatment plan Assess and treat swallowing difficulties and dysphagia Provide swallowing exercises and oral-motor therapy as appropriate Evaluate communication, speech, voice, and cognitive-linguistic abilities Provide communication therapy based on the client's needs Monitor tolerance and safety during therapy sessions Provide education to the client and family regarding swallowing safety, aspiration precautions, and therapeutic exercises Coordinate care with the physician, nursing staff, and other members of the care team Report changes or concerns promptly Complete accurate and timely clinical documentation Qualifications Active and unrestricted Georgia Speech-Language Pathologist license Current ASHA Certificate of Clinical Competence preferred Current CPR/BLS certification Experience treating adult patients with dysphagia, tracheostomies, and feeding tubes strongly preferred Home health or medically complex patient experience preferred Strong assessment, communication, and documentation skills Valid driver's license and reliable transportation Must complete all required background and employment screenings Schedule In-home speech therapy evaluation and treatment visits Flexible scheduling Visit frequency will be based on the established plan of care Immediate availability preferred
