Job Summary:
SLP shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
Responsibilities/Requirements:
Screen all residents in the facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Speech Therapy intervention is indicated. Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual. Reassess patients and caseload every 30 days to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated. Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment. Educate staff, family, and care givers as indicated. Identify needs for program development and implement programs according to company policies and procedures. Identify equipment needs of Speech Therapy Dept. and order equipment according to company and facility policy. Complete all required documentation according to guidelines specified in Documentation Manual. Coordinate in-services for facility staff as needed is determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents. Attend all facility required meetings. Promote profession within facility, including coordinating special events during National Speech Therapy Month. Attend Continuing Education courses specific to current position. Maintain professionalism and always follow all national/state association's regulations. Supervise student affiliates as directed.
Education and Licensure:
Must have a master's degree in Speech-Language Pathology from an accredited program. ASHA (CCC) Certification Active state license required.
Physical Requirements:
Ability to stand, sit, and move around for extended periods to evaluate and treat patients. Physical strength and dexterity to facilitate therapeutic activities, assist with transfers, and use equipment. Capacity to lift and position patients as needed during therapy sessions. Potential exposure to infectious diseases and human waste products. Prolonged periods of sitting and working on an electronic device.
Qualifications:
Must have a master's degree in Speech-Language Pathology from an accredited program. ASHA (CCC) Certification Active state license required.
