Days - Monday through Friday As a home health care speech language pathologist with Northwest Home Health and Rehab, your visits are tailored to your geographical location and schedule. Speech Language Pathologist Responsibilities Improves or maximizes the communication of the patient. Provides full range Speech Language Pathology Services as ordered by physician. Returns the individual to optimum and productive living within the patient's capabilities. Consults with physicians regarding change of treatment. Educate patients, caregivers and family members. Work closely and collaboratively with all members of the health care team. Inform the Clinical Manager of patient progress and clinical concerns. Record patient medical information in our electronic medical record system, Axxess. Speech Language Pathologist Qualifications A person who has a master's or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state of Illinois Two (2) years' experience, preferred.
