Old Bluegrass AVE, Louisville, KY 40215-1168 Job Description The Therapy Services Department provides physical, occupational, and speech therapy services to all residents at Bluegrass Hazelwood as well as appropriate individuals through Hazelwood Outpatient Specialty Clinic. The Therapy Services Department also works closely with the Nutritional Department in the development of programs. Staff in the department complete discipline specific assessments, perform treatments, develop mealtime, physical, and sensory support plans, train staff and monitor programs. All services are provided at the highest standard of professional practice, complying with standards by both federal and state regulations and facility policies. Required Education and Experience Graduate of an accredited Speech Therapy program. Must be licensed by the Kentucky Board of Speech Therapist Must be licensed by ASHA Preferred Education and Experience Preferred experience working with individuals with intellectual disabilities. Position Type/Expected Hours of Work This is a Part-time position, days and hours of work are typically 4:30pm-7:30pm Monday thru Thursday (not to exceed 50 hours per month). Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Assess all individuals on an annual basis to determine need for speech therapy. Conduct speech therapy assessments and analysis, complete all required documentation. Develop, monitor and train on learning based objectives. Participate in all assigned meetings. Complete cross-training with community providers. Develop, train and monitor all applicable communication, sensory, physical, and dining Plans. Provide staff training as assigned. Provide direct speech therapy services with individuals identified as needing those services. Shall provide services to meet the treatment needs of the specialty intermediate care (IC) clinic patients for services in field of care. Shall meet requirements of 902 KAR 20:410 Specialty Intermediate Care Clinics as warranted. Other duties as assigned by Therapy Director or Assistant Therapy Director Comply with all state speech therapy licensing requirements and regulations. Complete all required inpatient and outpatient billing. Learn and follow all policies and procedures. Supervisory Responsibility Supervises therapy support staff routinely. Travel This position may require travel to the Group Homes locations as well as community programs outside of Hazelwood Center when an individual is transitioning (i.e. SCL, day program, FHP, etc ).
