Job Description
VitalCaring seeks a compassionate Speech Language Pathologist to provide in-home evaluation and therapy for adult home health and hospice patients. You will complete assessments, develop individualized treatment plans, deliver evidence-based speech, language, cognitive, and swallowing therapy, and educate patients and caregivers. Collaborate closely with nurses, therapists, and physicians to coordinate care and support safe, goal-driven discharge planning.
