Develop individualized treatment plans based on residents' needs, goals, and medical conditions. Provide therapy to address issues such as dysphagia (swallowing), aphasia, dysarthria, voice disorders, and cognitive impairments. Monitor and adjust treatment plans regularly based on progress, changes in condition, or interdisciplinary input. Collaborate with nursing staff, physicians, occupational therapists, physical therapists, and dietitians to ensure coordinated care. Recommend and implement safe swallowing strategies and diet modifications to reduce aspiration risk. Educate residents, families, and staff on communication strategies, swallowing precautions, and use of adaptive equipment. Document evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory and facility guidelines. Participate in care planning meetings and contribute to interdisciplinary team discussions. Ensure compliance with state, federal, and facility regulations, including infection control and patient safety standards. Train staff on communication techniques and feeding assistance as needed. Support quality-of-life goals by helping residents maintain functional communication and safe oral intake.
