Description We are seeking a Home Health Speech Therapist (Speech-Language Pathologist) to join our team at AdvisaCare! In this role, you will work directly with clients to evaluate, develop, and implement individualized treatment plans addressing speech, language, cognitive-communication, and swallowing disorders. You will complete comprehensive assessments, establish goals of care, and provide therapeutic interventions designed to improve communication abilities and ensure safe swallowing function. Additionally, you will collaborate with interdisciplinary healthcare teams to ensure seamless care coordination and promote optimal patient outcomes. Responsibilities Conduct comprehensive speech and language evaluations to determine patient needs. Develop and implement individualized treatment plans in accordance with physician orders and agency standards. Provide therapeutic interventions targeting speech, language, voice, cognitive-communication, and swallowing disorders. Educate patients and families on communication strategies, compensatory techniques, and safe swallowing practices. Assess and document patient progress, modifying treatment plans as clinically appropriate. Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes. Communicate effectively with clients, families, caregivers, and physicians. Maintain compliance with all legal, ethical, and professional standards.
