OakMed West Home Nursing & Rehabilitation can help you with that! OakMed West is seeking compassionate, licensed Speech Language Pathologists (SLP) to deliver high-quality, patient-centered care directly to patients in the comfort of their homes. In this role, you will evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews —all within the comfort of their homes.
Compensation:
$85 per completed 1-hour patient visit
Key Responsibilities:
Evaluate patients to determine speech, language, cognitive-communication, voice, and swallowing deficits. Develop and implement individualized treatment plans based on patient needs and physician orders. Provide skilled speech therapy interventions in the home setting. Monitor patient progress and revise treatment plans as needed to achieve established goals. Educate patients, family members, and caregivers regarding treatment plans, safety techniques, communication strategies, and home exercise programs. Maintain accurate, timely, and compliant clinical documentation. Complete required assessments, evaluations, progress notes, recertifications, and discharge summaries. Collaborate with physicians, nurses, physical therapists, occupational therapists, social workers, and other healthcare professionals. Participate in case conferences and interdisciplinary care planning activities. Identify changes in patient condition and communicate findings to the appropriate healthcare provider. Promote patient independence, safety, and quality of life.
Requirements:
Graduate of an accredited Speech-Language Pathology program Current Speech Therapist (SLT) license in the State of Michigan Minimum of one (1) year of experience preferred (home health or clinical setting) Valid driver's license, reliable transportation, and auto insurance Ability to meet physical demands including lifting up to 25 pounds occasionally. CPR certification preferred
