What You'll Be Doing As a Speech Language Pathologist, you'll evaluate patients in their home environment and create individualized treatment plans designed to improve communication, cognition, memory, problem-solving, and swallowing safety. You'll work closely with patients, families, and the interdisciplinary team to deliver care that supports independence and improves quality of life. Responsibilities Provide in-home speech therapy evaluations and treatment visits. Assess speech, language, cognition, and swallowing function. Develop and implement individualized plans of care. Provide communication and cognitive training. Evaluate swallowing safety and feeding techniques. Educate patients and caregivers on strategies that support success at home. Collaborate with nurses, therapists, and physicians to coordinate care. Complete timely and accurate EMR documentation. Qualifications Current Minnesota Speech Language Pathologist License Required Valid driver's license and reliable transportation. Ability to work independently in a field-based setting.
