As a PRN/Per Diem Speech Language Pathologist (SLP) , you will have the opportunity to provide high-quality patient care to our residents in a skilled nursing and rehabilitation setting. Responsibilities:
Assess referred patients that require rehabilitation services Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions and potential for rehabilitation Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician Set realistic and achievable goals for patients Document and record patients' conditions Educate patients and families in an appropriate therapy method Evaluate, write weekly progress notes, recertification, and discharge documentation Collaborate with all disciplines to provide best patient-centered treatment and care Perform all other duties assigned by Director of Rehabilitation Maintain confidentiality,
HIPAA, and Federal Residents' Rights Qualifications/Requirements:
