We're seeking a highly motivated and independent Speech-Language Pathologist to join our team. Key Responsibilities Identify and meet patient goals through holistic assessment and functional reassessments. Conduct thorough evaluations using standardized procedures and point-of-service documentation. Develop and implement individualized, goal-oriented treatment plans. Document treatment progress and discharge summaries with accuracy and clarity. Collaborate with nursing staff, residents, and families to maximize patient outcomes. Provide weekend support as needed to ensure continuity of care and compliance with Medicare standards. Deliver evidence-based, patient-centered therapy that promotes independence and quality of life. Educate residents, families, and caregivers on therapeutic programs and home exercises. Participate in interdisciplinary team meetings and provide input on resident progress and care needs. Represent the rehabilitation team in facility meetings and collaborate with marketing/sales teams as required. Ensure residents are fully informed about services, procedures, and any associated risks, documenting all communication in the EMR. Maintain productivity levels consistent with facility expectations. Qualifications & Skills Master's degree in Speech-Language Pathology from an accredited program. Valid, active state license to practice Speech-Language Pathology. Clinical experience as an SLP (CFY candidates encouraged to apply). Knowledge of current treatment practices and evidence-based interventions. Familiarity with OBRA regulations, HIPAA, LCD, and third-party reimbursement guidelines. Compliance with ASHA standards and all state/national regulatory requirements. Strong interpersonal, verbal, and written communication skills. Professionalism, flexibility, and a commitment to continuous learning.
