Assess communication, cognitive-linguistic, and swallowing function. Identify learning needs, functional limitations, and rehabilitation goals. Collaborate with patients, caregivers, physicians, and the care team to develop individualized treatment plans. Deliver Exceptional Clinical Care Implement evidence-based speech-language pathology interventions in accordance with physician orders. Provide treatment for communication disorders, cognitive impairments, and dysphagia. Develop and implement therapeutic programs designed to maximize independence and functional outcomes. Continuously monitor patient progress and modify treatment plans as clinically indicated. Coordinate discharge planning and connect patients with appropriate community resources. Patient & Family Education Educate patients and caregivers using age-appropriate teaching methods. Train patients and families in communication strategies, swallowing safety, cognitive support techniques, and home programs. Evaluate understanding and adjust educational approaches as needed. Document educational interventions and outcomes. Ensure Safety & Quality Care Assess environmental and social factors that may affect patient outcomes. Identify clinical risks and implement appropriate safety measures. Adhere to infection prevention and control standards. Maintain compliance with agency, state, federal, and payer requirements. Case Management & Team Collaboration Serve as an active member of the interdisciplinary care team. Communicate effectively with physicians, nurses, therapists, and other healthcare professionals. Participate in team conferences and care coordination meetings. Manage documentation, scheduling, productivity standards, and service utilization responsibly. Support Organizational Growth Assist with onboarding and mentoring new team members. Participate in cross-training and professional development opportunities. Contribute to a positive, patient-focused culture. Perform additional responsibilities as assigned. Qualifications Required Master's Degree in Speech-Language Pathology from an accredited program. Current New Jersey Speech Therapy License. Minimum of one (1) year of experience in an acute care, subacute, rehabilitation, or home health setting. Strong interpersonal, communication, and customer service skills. Valid driver's license, reliable transportation, and willingness to travel to patient homes and care facilities. Ability to work independently while maintaining strong collaboration with the healthcare team. Preferred Home health experience. Experience managing complex adult and geriatric patient populations. Knowledge of home health regulations, reimbursement requirements, and documentation standards.
