Job Description

Duties
  • -Performs Patient assessments
  • Accurately assesses patient's appropriateness for home care, learning needs and progress toward goals.
  • Collaborates with patient, family, and physician to develop the plan of care for the patient.
  • Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
  • -Maintains safe environment for patients and co-workers:
  • Assesses social environment and adjust or provide education as needed.
  • Assess for potential clinical risks and adapt program as indicated.
  • Follows universal infection control measures.
  • -Implements treatment program to achieve designated goals:
  • Administer occupational therapy services per physician orders.
  • Instruct in communication and safe swallow, cognitive training as indicated.
  • Develop and instruct in therapeutic exercise program.
  • Implement program to restore function, and achieve maximum functional status.
  • Provides discharge planning and referral to resources as needed.
  • Communicate with physician, nurse, and other clinical disciplines as necessary.
  • Develops/implements/evaluates plan of care based on evidence-based knowledge.
  • Revise care plan as indicated within physician orders.
  • -Uses age appropriate teaching methods for patients and families:
  • Evaluates teaching plan and revises as necessary.
  • Documents patient education tools utilized.
  • -Demonstrates compliance with agency policies:
  • Assumes responsibility for Case Management.
  • Demonstrates knowledge of home care regulations and insurance reimbursement guidelines.
  • Assures compliance with regulatory requirements.
  • Follows agency documentation policies.
  • Shows evidence of interdisciplinary communication.
  • Ensures appropriate utilization of services.
  • Follows scheduling process.
  • Maintains visit productivity standards.
  • Participates in Team and IDT meetings.
  • Prioritizes schedule to accommodate patient's needs.
  • Travels to patient homes and facilities as indicated.
  • Complies with paid time off request process and weekend/holiday requirements.
  • -Assists with growing agency business:
  • Provides orientation to new employees.
  • Accepts other assignments as requested.
  • Cross trains to learn new skills.
  • Use of personal vehicle is required for home visits.
