Location:
Morristown, NJ Schedule:
Monday-Friday, 8:00 AM - 4:00 PM (No weekends)
Contract:
13 weeks
Pay Rate:
$45/hour
Position Summary:
We are seeking a licensed Speech-Language Pathologist to provide in-home therapy services for patients in a home health setting. The SLP will evaluate, plan, and deliver individualized treatment programs focused on improving communication, cognitive function, and swallowing abilities, while supporting patients' independence and safety at home.
Key Responsibilities:
Conduct comprehensive patient assessments including speech, language, cognitive, and swallowing functions Develop and implement individualized care plans in collaboration with patients, families, and physicians Provide direct therapy services focused on communication, cognition, and safe swallowing Monitor patient progress and update treatment plans as needed Educate patients and caregivers on communication strategies and swallowing safety Ensure accurate and timely clinical documentation in compliance with agency policies and regulations Coordinate care with physicians, nurses, and other interdisciplinary team members Support discharge planning and referrals to appropriate community resources Maintain patient safety and follow infection control standards during home visits Travel to patient homes as required
Required Qualifications:
Master's degree in Speech-Language Pathology from an accredited program Active New Jersey Speech-Language Pathology license Minimum 1 year of experience in acute, subacute, or home health setting Valid driver's license and willingness to travel for home visits Strong communication and patient care skills
Preferred Skills:
Prior experience in home health care Familiarity with interdisciplinary team-based care and case management
Pay:
