Job Description

About Us Family Care Certified Services is a leading Certified Home Health Agency (CHHA) providing high-quality, patient-centered care across New York. We are actively expanding our clinical team and seeking Occupational Therapists (OTs), Certified Occupational Therapy Assistants (OTAs/COTAs), Physical Therapist Assistants (PTAs), and Speech-Language Pathologists (SLPs) to support our growing patient base. Position Overview As a field clinician, you will provide skilled therapy services in patients' homes , helping them improve independence, function, and quality of life. This role follows CHHA and Medicare guidelines , including OASIS-based assessments and documentation . Scope of Work (CHHA Requirements) Conduct Start of Care (SOC), reassessments, and routine visits (as applicable by discipline) Complete and support OASIS documentation in compliance with DOH and CMS regulations Develop and implement individualized Plans of Care (POC) with physician oversight Deliver skilled therapy interventions per discipline Document all visits accurately and submit documentation timely Educate patients and caregivers on home safety, mobility, and independence Monitor patient progress and update care plans as needed Collaborate with RNs, case managers, and interdisciplinary team members Maintain compliance with CHHA, DOH, and Medicare guidelines Responsibilities by RoleOccupational Therapist (OT) Perform evaluations and establish treatment plans Focus on ADLs (bathing, dressing, mobility, upper extremity function) Supervise
OTA/COTA
services OTA (Certified Occupational Therapy Assistant - COTA) Provide therapy services under OT supervision Implement treatment plans and report patient progress Assist patients in improving functional independence and daily living skills Physical Therapist Assistant (PTA) Provide therapy under supervision of a Physical Therapist Assist with mobility training, strengthening, balance, and rehab exercises Monitor patient response and communicate progress Educate patients on fall prevention and safe mobility Speech-Language Pathologist (SLP) Evaluate and treat speech, language, cognitive, and swallowing disorders Develop individualized care plans Provide dysphagia and communication therapy Educate patients and families on communication and feeding strategies
QualificationsOT:
Active New York State Occupational Therapist License OTA (COTA): Active New York State OTA License NBCOT Certification (COTA) required Graduate of an accredited OTA program
PTA:
Active New York State PTA License Graduate of an accredited PTA program
SLP:
Active New York State Speech-Language Pathologist License CCCs preferred
All Candidates:
