(NDGCS) is seeking a dedicated and experienced Speech-Language Pathologist (SLP) to join a 252-bed Skilled Nursing and Rehabilitation Facility in Montrose, New York. This position offers the opportunity to work with residents requiring speech, language, cognitive-communication, voice, and swallowing therapy services in a collaborative rehabilitation environment. Position Responsibilities Evaluate, assess, and treat speech, voice, language, cognitive-communication, and swallowing disorders. Develop individualized treatment plans and provide rehabilitation services, counseling, and family education. Screen new admissions for swallowing safety and appropriate diet recommendations. Recommend positioning techniques, compensatory strategies, adaptive equipment, and referrals as appropriate. Participate in care plan meetings and discharge planning activities. Collaborate with nursing staff, physicians, dietary teams, residents, and families to ensure quality patient care. Maintain accurate and timely documentation of evaluations, treatments, progress notes, and outcomes. Monitor resident progress and adjust treatment plans as needed. Minimum Qualifications Current New York State Speech-Language Pathologist License. Minimum three years of supervised Speech Therapy experience. Strong clinical assessment, treatment, communication, and documentation skills. Ability to work effectively in a multidisciplinary healthcare environment. Preferred Qualifications Skilled Nursing Facility (SNF) experience. Experience with dysphagia management, geriatric populations, cognitive-communication disorders, memory care, or long-term care settings. Experience working in rehabilitation or post-acute care environments.
